Continuing Care

Support doesn’t end with one program.

Planning for what comes next begins during treatment. Explore how ongoing support can reflect your teen’s needs, progress, and everyday life.

Thoughtful planning. Continued connection.

Follow-up services, appointments, referrals, and insurance arrangements require confirmation.

Planning Begins During Treatment

Looking ahead.
At an individual pace.

Planning for ongoing support begins during treatment. As needs change, the team considers progress, safety, family support, and the appropriate next level of care.

The goal is to help families understand recommendations and prepare for the next steps.

Your next step is personal.

Continuing care is not the same for every teen. Recommendations may involve existing providers, services through Altara when available, or community referrals.

Appointments, services, and insurance arrangements must be confirmed.

What Continuing Care May Include

Support that reflects
what comes next.

Depending on needs and availability, the team may recommend one or more forms of ongoing support.

Outpatient therapy

Ongoing therapy when recommended for the teen’s needs.

Medication management

Follow-up with an appropriate prescriber when indicated.

Family support

Recommendations that include caregivers when appropriate.

Provider coordination

Coordination with existing providers, with appropriate consent.

Moving Between Levels of Care

As needs change,
support may change too.

A transition to less intensive care may be considered when clinically appropriate and services are available.

PHP

Structured daytime care

A higher level of scheduled support when needed.

Outpatient & community care

Follow-up based on individual recommendations.

Ask About Follow-Up

These are possible care options, not a required sequence. Not every teen follows the same pathway.

School, Family & Everyday Life

Care connects
with the life around it.

Continuing-care planning may consider school participation, family routines, relationships, and the support available outside treatment.

Coordination with caregivers, schools, and providers may occur with appropriate consent.

Academic services and accommodations are determined by the school and are not guaranteed.

Your Next-Step Plan

Know the recommendation.
Confirm the details.

Before a transition, ask the team what has been recommended and which arrangements still need confirmation.

  • Recommended services and level of care
  • Providers or services to contact
  • Appointments and availability
  • Insurance benefits and potential costs
  • Family participation and communication
  • Who to contact if needs change

Altara Follow-Up & Community Referrals

Different ways
to stay connected to care.

Through Altara

Follow-up when available

Continuing support through Altara may be available when appropriate. Services, appointments, and coverage must be confirmed.

Community Support

Coordination beyond Altara

The team may coordinate community referrals or communicate with existing providers when appropriate. External availability and insurance arrangements require confirmation.

Questions & Answers

Clear information.
Thoughtful planning.

Ask the team how continuing-care recommendations apply to your teen.

Contact Altara
Is follow-up through Altara guaranteed?

No. Follow-up depends on clinical suitability and available services. Community referrals may be coordinated when appropriate.

Can my teen keep their current therapist?

Coordination with an existing therapist may be part of the plan, with appropriate consent. Discuss recommendations, provider availability, and coverage with the team.

Can my teen transition from PHP to IOP?

Sometimes, when clinically appropriate and IOP services are available. The transition is based on individual needs and clinical review.

Will insurance cover continuing care?

Coverage depends on the plan, provider, and approved services. Benefits, authorization requirements, and potential costs require confirmation.

Who should we contact if needs change?

Ask the care team to clarify contact arrangements in the continuing-care plan. For an emergency, use emergency services rather than waiting for a routine response.

A Place to Begin Again

Let’s plan
what comes next.

Ask about continuing-care recommendations, follow-up options, and coordination with other providers.

Need urgent help?

Altara is not an emergency or 24/7 crisis service. Do not wait for an admissions response in an emergency.
