REQUEST AN ASSESSMENT

You don’t need
every answer.
Just a place to start.

Tell us how to reach you. We’ll help you understand the assessment process, explore your questions, and confirm current availability.

Submitting a request does not schedule an appointment or confirm enrollment.

WHAT HAPPENS NEXT

One step at a time.
A clearer way forward.

You don’t need to choose a program before reaching out. Start with a question, and learn what may come next.

  1. Send Your Request

Share your contact details and what you’d like information about. Keep your first request simple.

  2. Have a Conversation

Discuss the assessment process, your questions, and current availability with the team.

  3. Understand Your Options

Learn about possible next steps, including an assessment when appropriate and available.

A first conversation.
At your pace.

Your request is a first contact, not a clinical assessment. Appointment scheduling, program eligibility, insurance coverage, and enrollment require confirmation from Altara.

HELPFUL ANSWERS

A little clarity.
Before your next step.

It’s okay to have questions. Here are a few things to know about requesting a conversation with Altara.

Do I need to choose IOP or PHP before reaching out?

No. You can begin by asking about the assessment process. The appropriate level of care depends on individual needs, clinical assessment, and available services. You can also explore our programs before contacting us.

Can a teen submit a request?

Teens can reach out to ask general questions about care and what to expect. The team can explain how a parent or legal guardian may need to participate in assessment and enrollment, including applicable consent requirements.

Does this request confirm an appointment?

No. Submitting the form starts an inquiry. It does not schedule an appointment, establish a treatment relationship, or guarantee admission. The team will discuss possible next steps and current availability.

Can I ask about insurance?

Yes. You can ask general questions about insurance and payment. Please don’t send insurance cards or policy documents through this initial request. The team can explain the appropriate process for reviewing coverage. Learn about admissions and insurance.

What information should I avoid sending?

Please don’t include medical records, diagnoses, detailed treatment histories, identification documents, or other sensitive clinical information. Keep your request focused on contact details and general questions. Ask Altara how to share private information when needed.

Are programs currently available?

Altara’s programs are in development. Contact the team to confirm current availability, eligibility, care formats, schedules, and enrollment options.
